Out of the Mattress Testimonial Recording & Media Release Acknowledgment

By agreeing to this release, I acknowledge that I am voluntarily participating in a recorded testimonial video with representatives of Out of the Mattress, a nonprofit program sponsored by Build Up, Inc., a 501(c)(3) public charity (“OOTM”). I grant OOTM permission to use my name, likeness, voice, and any biographic or other information I provide in photographs, video, audio, or other digital media for print, web-based, or other publications, in perpetuity. I understand that these materials may be edited, altered, or combined with other content, and I waive any right to inspect or approve the final use. I acknowledge that all recordings are the property of OOTM and that I will not receive compensation or royalties for their use.

I represent that I have the right to enter into this release and that my statements are truthful, non-confidential, and do not infringe on the rights of others. I release and hold harmless OOTM, Build Up, Inc., and their affiliates, officers, employees, and agents from any claims related to this release. I understand that I may request that OOTM erase, forget, or cease use of my personal information at any time by submitting a request to legal@outofthemattress.org.
